Please contact the clinic location where you have received medical treatment. The clinic staff will be able to accommodate your request for medical records. Please remember to bring a valid photo ID.
If you would like to receive a copy of your psychotherapy notes, please complete the Authorization for Release of Psychotherapy Notes Form and return it to Tulane Doctors in one of the three ways listed above.
